    1. Replace *I am opento using different provider* in section 7 with * I am open to using different provider*
    2. There is no default input field assigned in *Contact Information* filed
    3. Tab function went from *First name* to *Referred by* instead of *Last name* fields.

    1. "Email:" field is not required in "Contact Information" section when you submit an empty form.
    2. "Referred by:" field is required in "Contact Information" section when you submit an empty form.
    3. Some mobile providers are missed in "Current Provider" list box at "Cell Phone Service" section.
    4. Option "Other" is missed in "Current Provider" list box at "Cell Phone Service" section.
    5. Button "Tab" works illogically (in free form not top-down left-right way).
    6. Typo in "IND" state abbreviation in "State" list box "Contact Information" section.
    7. State "HI" missed in "State" list box "Contact Information" section.
    8. State "NV" missed in "State" list box "Contact Information" section.
    9. There is unknown "BC" state in "State" list box "Contact Information" section.
    10. Do not need option "Other" in "State" list box "Contact Information" section.

    1."State" list box contains only 48 US states and 1 canadian province "BC".
    2."ZIP" field accept letters not just numeral.
    3. "Referred by" is required filed, but dosn't have asterisk.
    4. Section 1."Cell Phone Service", "Current Provider" list box dosn't include AT&T provider.
    5. Section 1."Cell Phone Service", "My monthly bill is approximately" accept letter not just numeral.

    TELECOM BUG REPORT
    1. Missing Cursor in First Data Entry Field
    2. Tab-order jumps from "First Name" to "Referred by"
    3. email is shown as required field - but possible to submit form wihout it
    4. Make Phone number DATA entry fiels auto-tabbed
    5. change all CHECK MARK boxes to RADIO BUTTONS
    6. Part 1. "Cell Phone Service" change all five CHECK mark boxes to RADIO BUTTONS
    7. For "months left on current contract" in in drop down menu add "MORE"
    8. Add DATA INPUT field for Minutes of plan
    9. For "My monthly bill is approximately:" invalid data (noin-digits) accepted
    10. For "Minutes on plan:" add entry data field.

    1. 'Email' input field is not implemented as required
    2. 'Reffered by' input field is implemented as required but it is not marked
    3. 'Email' input field allows invalid data (without'@')
    4. Single space symbol counts as the valid Email in 'Email' input field
    5. 'Submit' and 'Clear'buttons are different in size
    6. Text of 'Submit' button is bigger than text of 'Clear' button
    7. There is a Canadian state in 'State' dropdown menu (BC)
    8. NV and HA are not included to 'State' dropdown menu
    9. The 'Other' option in 'State' dropdown menu does not make sense because all states should be listed
    10. Tab order logic is not correct
    11. A spelling mistake in the 7th block ('opento')

    1. "Contact Information" section: tab order is mixed up and not logic
    2. "Contact Information" section: zip field can be fills with letters.
    3. "Contact Information" section: States in drop menu are not in alphabetical order.
    4. "Contact Information" section: E-mail field can be any letters or numbers without @.
    5. "Cell Phone Service" section: both alternate meaning check boxes can be checked - "I am happy with my current provider" and "I am open to using different provider".
    6. "Cell Phone Service" section: "My monthly bill" field can be fills with letters
    7. "Cell Phone Service" section: all of three check boxes can be checked together and no specific question for those alternative answers
    8. "Gas and Electric Services" section: misspelling "I am opento using"
    9. No default button
    10. When push "Clear" button there is an error "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!"

    Open Page ,observe :
    1. No default input focus called character.
    2. Tab order jump from first name to "refer by" then logical order
    3. First name field should`n be any numeric characters .alphabetic only
    4. Last name field should`n be any numeric characters .alphabetic only
    5."We will keep your information strictly confidential." should be mentioned in top of application .
    6. Cell phone service provider drop down tab missing ATT or "other"
    7. "My monthly bill is approximately" should not allow letters
    8. "My monthly bill is approximately" should accept decimal
    9. "Months left on current contract" drop down field 10 available instead 12 months
    10. "I like my handset" inappropriate field
    11. "Current Provider" i will suggest to have regular field instead drop down. with not complete list .

    Positive testing
    1.Failed to submit application
    -fill up required fields
    -push"submit" button

    Expected result
    -receive registration Email

    Actual result
    -No Email was reseived

    GUI
    1. " First Name " fields should not accept special characters and numbers.
    2. " Last Name" fields should not accept special characters and numbers.
    3. " City " fields should not accept special characters and numbers.
    4. " ZIP: " field should take only digits.
    5. There is no default field or button.
    6. " My monthly bill is approximately: " shouldn't accept alphabetic characters.
    7. " My monthly bill is approximately: " should accept a decimal numbers.
    8. " Current Provider: " drop bar should have " Other " selection.
    9. " State " drop bar is missing one State.
    10. " State " drop bar, " Other " selection should be at the end of the list or in the beginning.
    11. " Email " field should be " required field ".
    12. " Referred by " field not " Required field " we should be able to submit application with out that field.
    13. " Email " field should check for valid enter.
    14. Tab is not in order.

    1.In section Cell-phone service tab minute on plan all the numbers is in the spread(2,10,400)
    In the section contact information tab referred by accept numbers
    2. In the high speed internet section in the tab provider accept number also
    3.In the TV section, current provider listed not in alphabetic
    4. Gas and Electric section, current provider listed not in alphabetic
    5. When you push submit button, message error appears on the screen" could't find the www.energy-elecom.com

    1. Tab order does not follow logical pattern. After "First Name" field it jumps to "Referred by" field and only than to the "Last Name" field.
    2. In a "States" field Stated do not follow alphabetical order.
    3. "Months left on current contract" has only 10 months instead of 12.
    4. In a field 7 "I am open to using different provider" is misspelled with "opento".
    5. "Referred by" is a required field, however it is not marked like one.
    6. "Email" is marked as a required field however it is not mandatory to fill it up
    7. System accepts number characters in a "First Name" and "Last Name" fields.
    8. In a field 2 "Long distance calls included?" misspelled with "Long distance included?"

    2. Tabulating works incorrect, it switches after first field right in the down field in "Contact Information"
    3. In "Gas and Electric Services" in the bottom a word "opento" wrote together
    4. In "Contact information" field "Email" must be required field, but it is not
    5. The field "Referred by" is not required field, but is required filling out
    6. In "Contact Information" in field "State" is wrong sorting
    7. Button "Clear" shows error 17896
    8. In "Cell Phone Service" in field "My monthly bill is approximately" typing letters
