    Tab

    1. Tab order problem: after text field "first name" it jumps to "referred by", and then to "last name"
    2. tab order: after highlighted web element "Internet" it jumps to text field "total bill".
    3. tab order: cannot tab from "yes" to "no"
    check box


    in "international calls included", "long distance included" and "i am interested in Unlimited Calling" sections, and it is automatically "yes" selected pushing "tab" button.

    1.Change SELECT ONE to SELECT in all drop down menus
    2.Place (automatically) a blinking cursor at the beginning of the first data
    entry field
    3. make Phone number DATA entry fiels auto-tabbed
    4. Do not capitalize word provider in Current Provider - 6 cases
    5. point 1. Cell Phone Service change all five CHECK mark boxes to RADIO BUTTONS

    3. zip add not only numbers ( can be letters )
    4.cell phone area ( My monthly bill is approximately ) accept letters
    5."Months left on current contract:" shows 10 months instead 12
    6. zip can be more than 5 digits, if copy/paste
    7. "Gas and Electric Services" is misspelling " I am opento using different provider"
    8. Copy/Paste works everywhere. can be everything ( letters, characters, or digits )

    1. Tab order is corrupt in "Contact Information" section:
    "First Name" -> "Referred by" -> "Last Name"
    "Best Way To Contact Me" -> "Current Provider";
    2. States in "state" drop menu are not in alphabetic order, B before A;
    3. The option "other" in "States" drop menu should be the last in the list;
    4. ZIP text field allows to use letters;
    5. "Referred by" information is not marked as "Required" but is required in fact;
    6. "Current provider" should have AT&T in the list of providers or/and "Other" option;
    7. "My monthly bill is approximately" text field in section #1 accepts letters;
    8. Tab order in section #2 skips:
    -"Internet" and "TV" check boxes in "Bundled with"
    -"No" check boxes in "International calls included", "Long distance included", "I am interested in Unlimited Calling";
    9. Same issue in section #3;
    10. Same issue in section #4 with "Bundled with" question;
    11. "opento" grammar error, should be "open to" in section #7.

    1. No default input focus.
    2. Contact information section tab order is not logical.
    3. Contact information section state drop-down box has IND, expected result should be IN.
    4. Clear button smaller than Submit button.
    5. Clear button doesn't clear the form. Produces popup box: "Error 17896 - Invalid data request. Please call technical support or restart your computer"
    6. Contact information section ZIP field should not allowed to write letters but only digits.
    7. Section 1 "Minutes of plan" drop-down box missing minutes that needs to be include.
    8. Contact information section " Referred by" needs to check with " Indicates required field".
    9. Section 7 "I am opento using different provider" checkbox need to fix with " I am open to using different provider".
    10. Contact information section State drop-down box is not in alphabetic order.

    1.The "Street" text field accepts less than 255 characters.

    2. The "Email "
    text field accepts more than 255 characters.

    3. On the "State" list box "HI","DC" and "NV" items are missing.

    4. On the "State" list box the "IND" item should be changed to "IN" item.

    5. On the "State" list box the item "BC" and "Other" should be removed.
    6. The "ZIP" field should not allow letters only digits.

    7. Section # 1:The "C

    1.The web page don't have default input focus.
    2.Wrong tab order in "Contact information section".
    3."Contact information/ZIP code" field accept letters.
    4.Droplist in section "Cell phone service/Current Provider" don't have "Other" option.
    5."My monthly bill is approximately" in section Cell phone service, accept letters
    6. 11 and 12 months are missing in "Months left on current contract:" droplist, in Cell phone service section.
    7. High Speed Internet Service "/Type of service:" it possible to choose a service with a "Wireless Aircard" at the same time.
    8.Drop list in section "TV Service/Months left on plan:" don't have more than 24 months option.
    9.Checkbox "I am interested in Digital Phone Service (Unlimited Calling)" in "Digital Phone Service with Video Phone" section have different meanings with a section title.
    10. Misprint in the title of checkbox "I am opento using different provider" in "Gas and Electric Services" section.
    11.Droplist with States of USA in "Contact information section" have "Other" option
    12."Referred by:" field in in "Contact information section" don't have asterisk sybol that Indicates required field. After submitting a form with empty "Referred by:" text field, form show "* Please enter the name of the person you were referred by" message.
    13. "Submit" and "Clear" buttons has different sizes.
    14. After pushing "Clear" button, bowser show "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!" message.

    1. after filling up the" first name" TAB button is not moving cursor to the "last name" field,instead its jumps to " referred by"

    3. not all states are indicated in the state TAB, Nevada is missing
    4. Indiana state showing in 3 caracters
    5. after selecting the contact method TAB doesn't move to "referred by"

    7. Abbreviation of Indiana should be "IN" instead of "IND" in "State" drop-down list
    8. "ZIP" field should allow only five digits, no letters or special signs
    9. "Email" field should show input error message if email doesn't include "@" sign and end with ".com, .net. .ru, .ua, etc"
    10. "Phone" fields accepts any number input even "000-000-0000"
    11. Form cannot be submitted without filing out "Referred by" field even though it is not marked as "required field"
    Cell Phone Service Section
    12. "Monthly bill is approximately" field accepts letters

    16. If page is refreshed or reloaded All fields have been erased except for phone number.
    17. After reloading the page "Email" field has blue asterisks instead of a red one.


    18. In "Comments" Section there is no limit for amount of letters allowed in comment field.
    19. After writing random comment and copy pasting it several times to reach high number of letters in comments field, the page got completely non-functional. No error message, no response for any inputs.
    20. After page stopped responding, tried to reload the web page and it DOESN'T RELOAD, can't refresh, "go back button" also isn't responding, got completely locked up.
