    1. No default input focus on any of the forms
    2. "Tab" order is not correct - After "First Name" it jumps to "Referred by" instead of "Last Name"
    3. All field except for "Email" field should not allow input of special signs as "!@#$%^&*() etc."
    4. "State" drop-down list should start in alphabetical order with Alabama (AL) not "BC' which is misspelled "DC" (District of Columbia)
    5. "State" drop-down list should not include "Other"
    6. "State" drop-down list is missing Hawaii "HI", Nevada "NV".
    7. Abbreviation of Indiana should be "IN" instead of "IND" in "State" drop-down list
    8. "ZIP" field should allow only five digits, no letters or special signs
    9. "Email" field should show input error message if email doesn't include "@" sign and end with ".com, .net. .ru, .ua, etc"
    10. "Phone" fields accepts any number input even "000-000-0000"
    11. Form cannot be submitted without filing out "Referred by" field even though it is not marked as "required field"
    Cell Phone Service Section
    12. "Monthly bill is approximately" field accepts letters
    13. "Months left on current contract" field is missing "11" and "12" or "more" if contracts can be for more than 1 year
    Gas and Electric Services section
    14. "I am opento using ..." checkbox has misspelled "opento"
    15. "Clear" button is not functional, it shows error message "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!"
    16. If page is refreshed or reloaded All fields have been erased except for phone number.
    17. After reloading the page "Email" field has blue asterisks instead of a red one.


    18. In "Comments" Section there is no limit for amount of letters allowed in comment field.
    19. After writing random comment and copy pasting it several times to reach high number of letters in comments field, the page got completely non-functional. No error message, no response for any inputs.
    20. After page stopped responding, tried to reload the web page and it DOESN'T RELOAD, can't refresh, "go back button" also isn't responding, got completely locked up.

    1. There is no default input focus.
    2. Form can be submitted with only required data, absent energy-telecom data.
    3. Form can be submitted without email input but shows it is a required field.
    4. Form cannot be submitted unless "Referred by:" is filled in but does not show as a required field.
    5. Contact Information section TAB is not in the correct order.
    6. Contact Information section State drop-down box has "IND" instead of "IN".
    7 Contact Information section State drop-down box should not have "BC" as that is a Canadian Province.
    8: Contact Information section State drop-down box "Other" should come at the end of the 2-letter abbreviations, not in the current alphabetical order.
    9: Contact Information section First Name & Last Name: should not allow numerical or symbolic input.
    10. Section 1. Cell Phone Service, Current Provider: does not provide an "Other" option.
    11. Section 1 Cell Phone Service My monthly bill: is approximately: should not allow for letter input.
    12. Section 1 Cell Phone Service Minutes on Plan: do not follow a regular sequence of numbers.
    13. Section 1 Cell Phone Service Months left on current contract: missing numerical values of 11 & 12
    14. Section 2 Local/Long Distances/International Service Current Provider: is not in alphabetical order.
    15. Section 4 TV Service: Current Provider:is not in alphabetical order.
    16. Section 5. Home Security Service Current Provider is not in alphabetical order.
    17. Section 7. Gas and Electric Services Current Provider is not in alphabetical order.
    18. Section 7. Gas and Electric Services Current Provider drop-down menu has "Other" but no box is provided for input.
    19. Submit button and Clear button are different sizes.
    20. Clear button does not clear the form, shows a popup: " Error 17896- invalid data request".

    1. The Default Input Focus is not specified.
    2. Clicking on the "Clear" button shows an error message: "ERROR 17896 - invalid data request. Please call technical support or restart your computer!!!".
    3. It is possible to submit the review without any information in the "Email" text field, but it has the sign of required field.
    4. Buttons "Submit" and "Clear" have different sizes.
    5. Disable "Submit" button unless all the required fields are completed.
    6. "Contact Information" : TAB order moves from the "First Name" text box to the "Referred by:" text-box, instead of "Last Name".
    7. "Contact Information": remove "Other" menu item from the "State" drop-down box.
    8. "Contact Information": remove "BC" postal abbreviation from "State" drop-down box.
    9. "Contact Information": Hawaii (HI) and Nevada (NV) menu items are missing in "State" drop-down box.
    10. "Contact Information": replace "IND" postal abbreviation with "IN" in "State" drop-down box.

    17. "Cell phone Service": only 10 month specified in the "Month left on current contract" drop-down box.
    18. "TV Service": "HDTV" and "DVR" check boxes have different design (squared instead of rounded).
    19. "TV Service": add "Do not use TV Service" menu item to the "Current Provider" drop-down box.
    20. "Gas and Electric Services": replace "I am opento using different provider" with "I am open to using different provider"
    21. "Comments" input field does not have an indication of the letter limit.

    2. "Contact information" section: TAB order is not logical- input focus moves from the "First Name" field to the "Referred by". However, input focus should move from the "First Name" field to the "Last Name" field.
    3. "Contact information" section: "First Name" field should not accept symbols and digits.
    4. "Contact information" section: "Last Name" field should not accept symbols and digits.
    5. "Contact information" section: States are not properly sorted in alphabetical order in "State" list box.
    6. "Contact information" section: "State" list box should have 50 states instead of 49.
    7. "Contact information" section: "ZIP" field accepts digits and non-digits instead of only digits.
    8. "Contact information" section: "Referred by" text field does not have an asterisk symbol next to it. However, after submitting the form, the text field "Referred by" appears to be filled out as required field.
    9."Cell Phone Service" section: "Current provider" list box does not provide "Other" option to choose.
    10. "Cell Phone Service" section: "My monthly bill is approximately" field should not accept input of letters.

    13. "High Speed Internet Service" section: "Provider" text field should not be disabled.

    15."Home Security Service" section: "Current Provider" list box is not sorted in alphabetical order.
    16. "Gas and Electric" section: "Current Provider" list box is not sorted in alphabetical order.
    17. "Gas and Electric" section: the word "open to" is misspelled as "opento" in the checkbox label.
    18. "Submit" button should be disabled unless all the required fields in the form are filled out.
    19. "Submit" and "Clear" buttons at the bottom of the page are not consistent in their look: "Submit" button is larger than "Clear" button.
    20. Clicking the "Clear" button does not clear the form when an error message " Error 17896- invalid data request" is being shown.

    1. After the page is loaded there is no field with default input focus

    2. In section "Contact iformation" TAB order is improper, if begin to enter data from field "First Name:" and then press the TAB key input focus jumps to field "Referred by:", if begin pressing TAB key from any other field of this section the field "Referred by:" remain skipped and input focus jumps to next section

    3. There is no default button on the page

    4. LEFT-RIGHT; TOP-Bottom order doesn't present

    5. Some spelling errors present like "opento " in section #7 "Gas and Electric Services "

    6. In section #1 "Cell Phone Service" the FIELD "Email" specified like required field but nothing happens when we left it empty and submit a form (no warning message appear)

    7. In section #1 "Cell Phone Service" the FIELD "Referred by" behave like required field but it shouldnt (not specified by requirements)

    8. "Clear" BUTTON doesn't work when PUSHED the error pops up: "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!"

    9. In section #2 in labels "International calls included?" "Long distance included?" labels end up with question mark (?) better to use colon ( : ) character

    10. In section "8. Comments" we can manually transform size of the text field

    11. If we fill in only one section "Contact Information" and the rest of the form leaves blank we can still submit it, it's weird
