    Contact information

    1. "First Name" field accepts up to 30 characters instead of 31, accepts numbers and special characters.
    2. TAB order input focus moves "First Name" to "Reffered by" instead of "Last Name"/
    3. "Last Name" field accepted up
    to 30 characters instead of 31, accepts numbers and special characters.
    4. "Street Address" list box accepts numbers and special characters.
    5. "City" field accept up to 50 characters, digits and special characters.
    6. "State" list box "Other" is in the middle of the list, does not functional, has incorrect spelling name of some state.
    7. "State" dropdown list has non US state "BC", does not have "HI", "NV" US state, "IND" state must be renamed to "IN".
    8. "Zip code" field allows user paste special characters instead of accepting digits only.
    9. "Email" field accepts digits and special characters, accepts emails with out @sign.
    10. "Phone" field consists from 3 text fields(1st- code area, 2nd and 3rd-phone number areas) does not remove automatic from 1st area to another.
    11. "Referred by" field must be signed with special character (*) as a necessary requirement, because the "Referred by" field is a mandatory to fill out.
    12. "Referred by" accepts digits and special characters.

    1. "Current provider" list box missed "Other".

    1. Default focus input doesn’t presentment on the page.

    2. TAB order transition doesn’t work:

    from field “Best Way To Contact Me:” to field ”Referred by:”

    3. Field “State:” contain only 49 from 50. One state "IND" has 3 letters instead of 2.

    4. Field "Email" marked "*" (required field). If push the button "Submit" the prompt window (red table) doesn't mark this field with "*". And field "Referred by:" became required.

    5. Section "3. High Speed Internet Service" when you check the box "Wireless Aircard" it disables field "Provider:" but doesn't clean up it.

    6. The TAB order transition doesn't work inside the horizontal survey lists on the:

    Section "2. Local/Long Distances/International Service" fields: "Bundled with:" "International calls included?" "Long distance included?" "I am interested in Unlimited Calling:"

    1. There is no default input field
    2. TAB order is not functioning properly. After "First Name" it goes to "reffered by".
    3. In the "High speed Internet Service" section TAB order skips "Provider" text field.
    4. "First Name" text field accepts numbers.
    5. "First Name" text field accepts special characters.

    7. "Last Name" text field accepts special characters.
    8. "State" drop down box has irrelevant options, such as "BC", "Other", "IND".
    9. "ZIP" text field accepts letters.

    11. When submitting an empty apllication, asterisk by "Email" text field remains blue. (The rest turns to red).
    12. When submitting an empty application, an error message about blank required fields is displyed. An error about missing Email is no thtere.

    14. "ZIP" code text field accepts unexisting ZIP-code upon submitting. I tried "00000"

    1. "First name" field may have symbols and digits.
    2. "Last name" field may have symbols and digits.
    3. TAB from "First Name:" field of immediately jumps to "Refered by:" field, and then in the correct sequence.
    4. "State:" column does not contain not alphabetical order.
    5. In "State:" column HI is missing.
    6. In "State:" column "BC" and "OTHER" are excess.
    7. "State:" list boxshoul be deleted. There are only 50 states and all must be listed.
    8. Change in "State" column IND to IN.
    9. "ZIP:" field allows letters.
    10. "Email:" field accepts emails without "@" sign.
    11. The "Submitt" button completes the dispatch of the questionnaire without completed mandatory the "Email:" field.
    12. "Referred by:" haven't been shown as required field.
    13. Two concepts are simultaneously impossible: "I am happy with my current provider" and "I am open to using different provider."
    14. "My monthly bill is approximately:" field may have symbols and digits, is necessary only digital.

    18. Remove "DVR," this isn't a service, it's a device.
    19. No messages when entering information incorrectly.

    2. "Contact Information": Left -> Right and Top -> Bottom tab order is not being followed.
    3. "First Name" and "Last Name" fields accept digits.
    4. "State" drop-down list contains Canadian Province British Columbia "BC".
    5. "State" drop-down list: Indiana state is abbreviated as "IND".
    6. "ZIP" data input field accepts letters.
    7. "Contact Information": user is unable to submit the form until the "Referred by" text field is filled in.
    8. "Email" text field is marked as required, though the form can be submitted without filling in that field.
    9. "Email" text field: email address can be submitted without @ character.

    1. Button 'Clear' at the botton of application doesn't work. (Functional)

    Steps to reproduce:
    - go to www.energy-telecom.portnov.com

    - fill out the application with a data

    - push 'Clear' button at the botton of application

    As the result : Error 17896 - Invalid data request. Please call technical support or restart your computer!!!


    2. At the botton of the application buttons 'Submit' and 'Clear' have different sizes. Should be the same. (GUI)

    3. There is no default input option.

    4. The web application doesn't have any navigation options. Disabling
    navigation is confusing and frustrating to users, and can negatively impact user satisfaction and task completion. (GUI)

    5. There is no Homepage button or link. (GUI)
    6. There is no Search option.


    Contact information form

    1. Field ' First name ' is accepting numbers, which is invalid data. (Functional)
