    Energy-Telecom GUI Bug reports

    1. Favicon (website icon) is missing.

    2. "Contact information" field - "State" list box - The states names are not in alphabetical order.

    3. "High speed Internet service" field - The checkboxes are not aligned and have different forms (square and round).

    4. "Digital Phone Service with Video Phone" - Checkbox is not aligned with other page content.

    5. "Gas and Electric Services" field - "I am opento using different provider" checkbox text has grammar mistakes.

    6. "Submit" and "Clear" buttons have different size.

    1. Submit and Clear buttons are enabled without any provided entry
    2. No default input field
    3. 2nd TAB selection out of the logic
    4. "State" - Abbreviation for ID state is incorrect. Listed as IND
    5. "Best Way To Contact Me" - split to 2 rows
    6. Cell Phone Service - " Minutes on plan" - dropdown choices are not consistent
    7. Cell Phone Service - "Months left on current contract" - no option for 11 months
    8. Cell Phone Service - "Number of phones on plan" - dropdown of choices are not logical
    9. Cell Phone Service - "Current provider" - names are not in alphabetical order and no Other option
    10. Cell Phone Service - "My monthly bill is approximately" - dot for cents separation does not work

      1. Default input focus is absent.
      2. "Submit" button should be disabled till all required fields are filled out.
      3. "Clear" button doesn't work.
      4. Incorrect TAB order - after "First name" field should be "Last name" instead of "Referred by".
      5. Label "International calls included" in section 2 should end up with colon instead of question mark.
      6. Label "Long distance included" in section 2 should end up with colon instead of question mark.
      7. Misspelled word in section 7 - must be "I am open to using different providers", not "I am open to using different providers".
      8. Alphabetical order is violated in "State" list box.

        1. When just open the form default input focus doesn`t work
        2. After pushing Wireless Aircard, on the button "Provider" default input focus doesn`t work
        3. After First Name box Tab goes to Referred by box
        4."I am opento using different provider" the word opento is misspelled
        5. There is missing information about date when the form is completed or deadline date
        6. I field 2 there are question marks instead of colons

          Contact information:
          1. The fields (First Name, Last Name, City) designed for letter entries only accept digital entries
          2. The field State:
            1. items are not in alphabetic order
            2. The Indiana State abbreviation is wrong (must be IN)
            3. Missing states: HI, NV
            4. The Other item must be the last on the list
          3. The ZIP field accepts erroneous digital entries
          4. The items in the Best Way to Contact Me field are not in alphabetic order
          5. The TAB button is not working logically

            1. Default input focus is missing.
            2. Contact information: Tab order is not logical from "first name" field jumps into "referred by" field
            3.Contact information: In the State field in the drop-down menu, the states are not in the alphabetical order
            4. Contact information: Zip code field accepts alphabetic input
            5.Cell phone service: "current provider" is missing "other" option in the drop-down menu
            6. Cell phone service: in the drop-down menu "Months left on current contract" field is missing 11 and 12 months
            7. Local/Long Distances/International Service: Current Provider field: in the drop-down menu the providers are listed not in alphabetical order and the menu does not drop down but goes up
            8. High-Speed Internet Service: The checkboxes are not aligned and have different forms (square and round)
            9. TV Service: in the drop-down menu the providers are listed not in alphabetical order
            10. TV Service: The checkboxes have different forms (square and round)
            11. "Current provider" in sections 4, 5, 7 are listed not in alphabetical order
            12. Checkboxes through the application should be the same format square or circle

              Energy-Telecom GUI Bug reports.

              1. Default input focus is missing.

              In the "Contact information" box we have such bugs:

              1. In the text field "First name", "Last name" and "City" we can write numbers.
              2. In the text field "ZIP" we can write letters
              3. When we change data input field with TAB button it goes not logically.
              4. In data input field "State" states goes not alphabetically, section "other" should be at the end of the list.

              In the "Cell Phone Service" box we have such bugs:

              1. In data input field "Current Provider" we have such provider "Nextel" which does not exist.
              2. In data input field "Current Provider" providers goes not alphabetically.
              3. In the text field "My monthly bill is approximately" we can write letters.
              4. In data input field "Months left on current contract" only 10 month instead 12.

              In the "Local/Long Distances/International Service" box we have such bugs:

              1. In data input field "Current Provider" providers goes not alphabetically.
              2. In the title "Total Bill" we do not need capital letter in word "Bill".
              3. In the title "International calls included?" and "Long distance included?" must be without question mark.

              In the "TV Service" box we have such bug:

              1. In data input field "Current Provider" providers goes not alphabetically.

              In the "Gas and Electric Services" box we have such bug:

              1. In data input field "Current Provider" we have such provider "Edison" but must be "ConEdison".

                  Energy-Telecom bug report:

                  1. There is no field that gets default input focus.
                  2. Enabled "submit" button when text fields are empty.
                  3. There is no option to choose "other" in the "Cell phone service" field-> "Current provider" label.
                  4. There is a mistake in "Gas and Electric services" field in the check box "I am open to using different provider" ("open to" is written together").
                  5. There is BC state in the "Contact information" field->label "State". This state doesn't exist.
                  6. Contact information field: user can type 6 digits in the ZIP field when it's supposed to accept only 5.
                  7. "Clear" button doesn't work.

                    1) Default input box in disabled in the "First Name" field of the "Contact Information" table.

                    2) Tab moves input field in the disorder, from the "First Name" field to the "Referred by" field instead of moving to the "Last Name" field.

                    3) The State abbreviations are not listed in the alphabetical order in the "Contact Information" table.

                    4) Provider names are not in the alphabetical order in the "Current provider" field of the "Cell Phone Service", "Local/Long Distances/International Services", "Home Security Services", "Gas and Electric Services" sections.

                    5)The tab order is violated in the in the "Local/ Long Distance/International Service" section. The tab moves input field from the top to bottom missing the "TV", "Both", "No" radio buttons.

                    6) Default values are not provided in the "Contact Information" section.

                    7) The tab moves input field from the top to bottom missing the "Dial-up", "DSL", "Provider", "Phone", "Both" radio buttons in the "High Speed Internet Service" section.

                    8) The tab moves input field from the top to bottom missing the "Phone" and "Both" fields in the "TV Service" section.

                      1. Default Input Focus is missing.

                      2. Tab Order is incorrect:
                      a) Jumps from "First Name" text field to "Referred by" instead of "Last Name".
                      b) Tab Order is missing at "Provide" data input field.

                      3. Input Focus is disabled at "Provider" data input field.

                      4. "Clear" button doesn't work.

                        Energy-Telecom bug report:
                        1.The absence of the default field.
                        2. Incorrect TAB- order in the "Contact Information" area. (First name - Reffered by)
                        3. Non alphabet order of the State Abbreviations in the drop-down list of the State option of the "Contact Information" area
                        4. Disabling/enabling of the Provider field in the "High Speed Internet Service" area is not viewable
                        5. Disabling/enabling of the "My monthly bill" field in the "Home Security Service" area is not viewable
                        6. Misspelling of "I am open to" (I am opento) in the "Gas and Electric Services" area
                        7. Field "Referred by" is not marked with requirement sign however the System requires to fill out this field to submit.
                        8. Radio buttons("Cable", "Dial-up", "DSL") and Checkbox "Wireless Aircard" are located in disorder ("Wireless Aircard" should bu right under "Cable")
                        9. Buttons "Submit" and "Clear" are located too close to each other.

                          1. Default pres "Enter" send to unknown webpage;
                          2. "Best way to contact me" split in 2 rows;
                          3. Check box in "Cell phone service" works for both checks;
                          4. "I last upgraded my phone(s) approximately:" - split in 2 rows and has grammar mistake;
                          5. "High Speed Internet Service:" provider text field is disabled unless you will check the check box "wireless air card";
                          6. "I am interested in Digital Phone Service (Unlimited Calling)" - does not have dot;
                          7. "Submitting" button is inactive.

                            Energy-Telecom GUI Bug reports:

                            1. When I want to start typing - default input focus is missing.

                            2. Tab order doesn't work properly - it goes from the field 'First name' to the field 'Referred by'.

                            3. Tab order doesn't work properly - it goes from the field 'Best Way To Contact Me' to the field 'Current provider'.

                            4. The label 'Best Way To Contact Me' is divided into 2 lines.

                            5. In the field 'States' the word 'Order' in the wrong order.

                            6. When I push 'Clear' button I get an error message.

                            7. In the field 'Months left on current contract' - on the list only ten months.

                            8. In the label 'I am opento using different provider' is a spelling error.

                              * In the line "International calls included?" input focus moves from the radio button "Yes" to the radio button"Yes" in the line "Long distance included?" and skips over the radio button "No" in the line "International calls included?"
                              * In the line "Long distance included?" input focus moves from the radio button "Yes" to the radio button"Yes" in the line "I am interested in Unlimited Calling:" and skips over the radio button "No" in the line "Long distance included?"
                              * In the line "I am interested in Unlimited Calling:" input focus moves from the radio button "Yes" to the menu item "Current Provider:" in the chapter #3 and skips over the radio button "No" in the line "I am interested in Unlimited Calling:"

                              2. The following fields accept both: letters and numbers instead of only numbers:
                              * Chapter "Contact Information", text field "ZIP"
                              * Chapter #1, text field "My monthly bill is approximately:"


                              3. System doesn't check the information entered into the text field "Email" and accepts all text and numbers without symbol "@", although this text field is a required field.
