    1."Email" text field is not indicated as "Required" field (expected required field).
    2. Remove "Referred by" text field from "Required" field.
    3.Tab order violation: input focus moves from "First Name" text field to "Referred by" text field ("Last Name" text field is expected).
    4."First Name" text field accepts up to 30 chars instead of 31 as specified in data requirements.
    5."Last Name" text field accepts up to 30 chars instead of 31 as specified in data requirements.
    6."Street Address" text field accepts up to 100 chars instead of 255 as specified in data requirements.
    7.Remove "BC" list item from the "State" list box.
    8."State" list box: "IND" list item is spelled incorrectly ("IN" expected).
    9.Remove "Other" list item from the "State" list box.
    10."State" list box: "HI" and "NV" are missed list items.
    11."ZIP" text field accepts invalid chars (letters), digits only expected.
    12."ZIP" text field accepts up to 100 digits, 5 digits only expected.

    22. “clear” button does not clear page

      Energy-Telecom GUI bugs report:
      1. TAB order is inconsistent. After “First name” field it jumps to “Referred by” field instead “Last Name” field.
      2. No default input focus assigned on the form.
      3. Labels “International calls included” and “Long distance included” are ending with question mark (?) instead of colon (.
      4. “State” list box violates common rule of State input for State of Indiana (IND).
      5. “State” list box includes one non-US state (BS).
      6. “State” list box is missing two states (NV, HI).
      7. “Zip” text field allows to type in letters, whereas digits expected.
      8. “First Name” and “Last Name” fields allow to type in digits, whereas letters expected.
      9. “Months left on current contract” field allows input of up to 10 months only, whereas up to 12 months input expected.
      10. Among cell phone providers list box one of the biggest provider At&t is missing.
      11. “Minutes on plan” list box is inconsistent in terms of minutes’ increments (30, 200, 400, 450 etc.).
      12. “Number of phones on plan” list box includes unrealistic amount of lines possible on the plan (up to 100), whereas maximum possible is up to 10 lines for major carriers.
      13. “I am opento using different provider” label has spelling error (opento).

            1. The „E-mail” text field doesn’t warn about incorrect e-mail format.
            a. Type an incorrect email format in the “E-mail” field and push the “Submit” button; for example: “kkk”,
            Observation: The “E-mail” text field submits an incorrect email format.
            2. The “E-mail” text field accepts up to 1172 characters instead of 255.
            3. Data from “Contact Information” section are submitted in spite of leaving the “Email” field empty, which is required.
            a. Fill out all fields in the “Contact Information” section except for the “Email” field.
            b. Push the “Submit” button.
            Observation: The form submits data without a customer e-mail address what is required.
            4. The “First Name” and the “Last Name” fields accept up to 30 characters instead of 31 what is required.
            5. The “Street Address” field accepts up to 100 characters instead of 255 what is required.
            6. The “State” dropdown list contains 48 states instead of 50. It skips “HI” and “NV”.

            9. The “Referred by” field is not required but a customer needs to fill it out to submit the form.

            Observation: The form isn’t submitted. The message is displayed “Please enter the name of the person you were referred by”.

                My checklist to the Energy-Telecom Online Form. Tested on OS Windows 10

                1. The label “International calls included?”: add a colon character after “?”.
                2. The label “Long distance included?” doesn’t end up with a colon character.
                3. Dropdown list “State”: there are 49 items (states) only.
                4. Dropdown list “State”: remove “Other” item.
                5. Dropdown list “State”: all items have to be enumerated in the alphabet order.
                6. “Energy-Telecom Service Review” form: there is no any field that gets default input focus
                7. The text field “Referred by” are not defined as a required field. If you missed this text field and pushed the “Submit” button you’d see the notification “Please enter the name of the person you were referred by”.
                8. When you push the button “Clear” you will find out it reports error notification “17896. Invalid data request. Please call technical support or restart your computer!!!”.
                9. It’s not possible to renew the page after filling some text fields - just pop-ups the notification with warning you may lose the information you’ve typed in. When you push the button “Confirm” nothing happens.
                10. There is incorrect Tab order in the block of text fields “Contact information”.
                11. There are different font sizes on the buttons “Submit” and “Clear”.
                12. There is no defined format for the text field “My monthly bill is approximately” in section 1 “Cell Phone Service”.

                    A few bugs i found :

                    After filling personal info tabs the button «submit» is still active while the main survey (questionnaire) was not filled.

                    Labels “International calls included” and “Long distance included” end with question mark (? )instead of colon (.

                    In section 5 after you hit checkbox «I do not have home security services today, but I am interested in getting more information» the tab «My monthly bill is approximately» is still active .

                        1. When you push the "Enter" button, interface is not consist of having "Default Button".
                        2. The select box of "Best way to contact me" does not have "Indicate required" sign.
                        3. In the select box of "State" does not have "Selete" word notification.
                        4. User can selete both of the check boxes of "I am happy with my current provider" and "I am open to using different provider" same time, which is mutual contradiction.
                        5. When you selete from the selete box of "Number of phones on plan", if you selete option "More", there is no more "Text Field" where you could write down your amount.
                        6. When you selete from the selete box of "Minutes on plan", If you selete option "Other", there is no more "Text Field" where you could write down your amount.
                        7. When you selete from the selete box of "I last upgraded my phone(s) approximately", If you selete option "More", there is no more "Text Field" where you could write down your amount.

                              2. Tab order is not logical- after clicking on “First Name” text field and pressing TAB, cursor is moved into “Referred by” text field instead of “Last Name” text field
                              3. “Email” label should not contain asterisk, since text field does not require data input
                              4. “Referred by” label should contain asterisk, since text field does require data input
                              5. “State” dropdown list should not contain “BC” item, since it is not a state. Item should be removed
