    #1.
    Severity: Serious.
    Summary: Numbers and symbols can be accepted in the “First name” text field.
    Steps to reproduce: Input numbers and other symbols in the “First name” filed box.
    Push “Submit” button.
    Actual result: A name is accepted and saved.
    Expected result: Only name with letter symbols and hyphen symbol can be accepted and saved. An Error message should appear with numerical and symbolic input.

    Severity: Serious.
    Summary: A button “Clear” at the end of the form , if pushed shows an “Error” message.
    Steps to reproduce: Push the button “Clear” at the end of the form. Observe.
    Actual result : An error message “Error 17896_Invalid data request. Please call technical support or restart your computer!!!”.
    Expected result: All text field in the form being cleared. Optional warning message confirming an action to appear. Example: “Are you sure you want to clear form?”

    Severity: Serious.
    Summary: An “Email” text field has an optional feature even though indicated as required filed.
    Steps to reproduce: Leave an “Email” text field empty. Push “Submit” button.
    Actual result: An error message doesn’t appear after submitting an empty “Email” field.
    Expected result: An error message to appear mentioning that “Email” filed is a required field.

    #4.
    Severity: Serious.
    Summary: In the 1. Cell Phone Service section both options “I’m happy with my current provider” and “I am open to using different provider” can be selected, which defeats the purpose of questionnaire.
    Steps to reproduce: Check both check boxes.
    Actual result: Both answers are accepted.
    Expected result: If first answer chosen, a check mark should not be allowed for the second answer and vise versa.

    Severity: Serious.
    Summary: Zeros only can be accepted as an input in the “ZIP” text field.
    Steps to reproduce: Enter all zeroes in the ZIP text field. Push “Submit” button.
    Actual result: An answer is accepted.
    Expected result: An error message to appear.

    #6.
    Severity: Serious.
    Summary: In the section 1. Cell Phone Service, “Minutes on plan” text field an additional text field doesn’t appear for a customized input if a list item “other” chosen from the list drop down.
    Steps to reproduce: Choose “ Other” item in the “minutes on plan” text field.
    Actual result: Answer “Other” is accepted.
    Expected result: An additional text field to appear for a customized input.

    #7.
    Severity: Critical.
    Summary: Only 10 months(or 12 options) are available from the drop list in section 1. , “Months left on current contract”.
    Steps to reproduce: Push arrows on the “Months left on current contact” to unfold drop list.
    Actual result: only 12 options available to choose from.
    Expected result: A customized text filed for any numerical value input rather than a drop list. (Unless all contracts are maximum of 10 months long).

    Severity: Serious.
    Summary: An “email” text field accepts an email address without @ sign.
    Steps to reproduce: Type in any letters in the “Email” text field without @ sign.
    Actual result: An input is accepted.
    Expected result: En “Error” message, notifying to use correct email address format with @ sign.

    #9.
    Severity: Minor.
    Summary: An unneeded “Other” list item at the “State” drop list filed.
    Steps to reproduce: Push arrows on “State” drop list button.
    Actual result: “Other” option is available for the answer.
    Expected result: Only states abbreviations to be available to choose from.

    #10.
    Severity: Enhancement .
    Summary: Missing a space after word “open” in the section 7. In the sentence “I’m opento using different provider”.
    Steps to reproduce: Observe section 7 of the Review.
    Actual result: Words “open” and “to” are not separated.
    Expected result: A space between words “open” and “to”.
