    1. In "Contact Information" fillable fields, TAB is not in order (goes from "First Name" to "Referred By", when should go to "Last Name").
    2. "State" list box not in alphabetical order ("BC", then "AL").
    3. No default input focus when page initially loads.
    4. When using push button "Submit", does not highlight "Email" as requires field, if text field is empty, even though it is a required field, as indicated by the asterisk.
    5. When using push button "Submit", error message "* Please enter the name of the person you were referred by" appears, even though text field is not identified with asterisk as required field.
    6. Ability to enter letters (non numerical characters) in text field "My monthly bill is approximately", even though should be only numerical characters, as in other text fields on page identifying numerical identifiers.
    7. In list box "Months left on current contract", list only goes to 10 possible months; should be more.
    8. In list box "Number of phones on plan", list goes all the way to 100; should be less.
    9. Check box titled "I am opento using different provider" is misspelled; should be"I am open to using a different provider".
    10. List box "Current Provider" no in alphabetical order.

      1. Tab order is not logical.
      2. System allows you to input numbers and special characters in the Name, Last Name, Street Address, and City input field.
      3. State drop box is not in the alphabetic order (BC before AL).
      4. State drop box list includes Canadian province (BC), missing DC, HI and NV,has Indiana abbreviation incorrect (IND instead of IN) and has "Other" listed.
      4. E-mail is highlighted as "Required field", but does not show as one when submitted.
      5. "Referred by" field is not highlighted as "Required field", but is showing error in the header of the page when nothing is entered when submitted.
      6. "Referred by" is not marked with the red asterisk when submitted without data.
      7. System allows you to check and submit all the check boxes in "Cell Phone Service" section.
      8. "Monthly bill" allows you to input letters and submit it without error.
      9. Misspell at the end of "Gas and Electric Service" section (opento instead of open to) and no period at the end of the sentence.
      10. Pushing "Clear" button gives an error message "Error 17896 - invalid data request. Please call technical support or restart your computer!!!"

        1. Contact Information- TAB order doesn't work in logical manner. After "First name" field it goes to the "Referred by" instead of "Last name" field.
        2. Contact Information - "Referred by:" does not specified as required field, but if empty - system shows error.
        3. Contact Information - "State" - "BC" should be removed from list as a province.
        4. Contact Information - "State" option "other" should be removedl.
        5. Contact Information - "State" checkbox has only 48 states instead of 50.
        6. Cell Phone Service - "Months left on current contract:" has only 10 month available, but should be 11.
        7. Cell Phone Service - can choose more than one option at time.
        8. High Speed Internet Service - "Type of service" "Wireless Aircard" check box type is different from the all other options.
        9. High Speed Internet Service - "Type of service" can choose more than one option. Wireless Aircard can be always chosen + Cable OR Dial-up OR DSL.
        10. Gas and Electric Services - "Open to" misspelling (no space between them).
        11. Buttons "Submit" and "Clear" have different type of font size.

          1) "TAB" button goes in illogical order.
          CONTACT INFORMATION: 2) BC state are perfomed in US list. There is no such state.
          CONTACT INFORMATION: 3) State "IND" has 3 letter. There must be 2 letters in state abreviation.
          CONTACT INFORMATION: 4) "Email" field is not shown as required field.
          CONTACT INFORMATION: 5) List of states is not in alphabetical order.
          CONTACT INFORMATION: 6) Needs to remove "Other" from the list of states.

          GAS and ELECTRIC SERVICES: 10) Misspell in the word "opento".

                1. TAB order is not working properly (input focus moves from "First Name" to "Reffered By" instead of "Last Name"
                2. The field for default input focus is not specified
                3. Text field for "State" is not in ABC order
                4. "Enter" button is not working properly (Pushing "Enter" button after entering text in a text field does not actually allow to enter the text and move automatically to the next field)
                5. After choosing "Way to contact" input focus disappears
                6. Input focus disappeared every time after selecting any Text field from drop down menu
                7. Ability to check both boxes in "Current provider" field instead of one
                8. Ability to enter text in "Monthly bill" field instead of numbers only

                      3. "First name" field allows to enter numbers

                      5. "Current provider" label in the Section 1 doesn't end up with colon character
                      6. TAB order doesn't work in the Section 4. It moves from "Internet" box to "Number of receivers" field instead from "Internet" box to "Phone" box
                      7. When I push "Submit" button there is no indication about error: "Please enter your email" among other errors
                      8. When I push "Submit" button "email" label has a blue asterisk instead red
                      9. "Clear" button doesn't work to clear information
                      10. "Months left on current contract" field has options of maximum of 10 months instead 12

                        b. The tab function doesn’t work as design from the “First Name” field.
                        c. Remove “BC” and “Other” from the States drop down list.
                        d. Add “HI” and “NV” to the State drop down list.

                        a. “The Minutes on plan” is not in sequential order.
                        b. “Months left on current contract”: add 11 and 12.

                        Contact information:
                        1. “First Name” text field need to accept 31 characters.
                        2. “Last Name” text field need to accept 31 characters.
                        3. “City” text field need to accept 50 characters.
                        4. “Zip” text field accepts letters.
                        5. "Street address" text field should accept 255 characters.
                        6. The tab function doesn’t work as design from the “First Name” text field.
                        7. Email” field should display as required and does not.
                        Step to reproduce:
                        a. Open http://www.energy-telecom.portnov.com/qa/
                        b. All required fields should be not filling out.
                        c. Push the “Submit button”- observe.
                        • There is no “* Please enter your Email address” error message appears along with other required fields.
                        • An asterisks next to “Email address” text field is blue not red.
                        8. “State” drop down list:
                        a. Need to remove “BC” and “Other”
                        b. “HI” and “NV” are missing.
                        c. Replace “IND” to “ID”.

                          4. Labels with a question marks should end up with the colon character.

                            2. Section 1 "Contact information": name of the states in the "State" list box are not in alphabetical order.
                            3. Section 2 " Local/Long Distances/International Service": Menu item "I am interested in Unlimited Calling" has colomn( in the end instead of question mark (?)
                            4. Section 3 "High Speed Internet Service": check box "Wireless Aircard",located under the "Type of service" menu item, has a square shape insted of round.
                            5. Section 4 "TV Service":check boxes "HDTV" and "DVR",located under the "I currently have" menu item, have a square shape insted of round.
                            6.Sections 5,6 and 7 have square check boxes instead of round
                            7. Section 7 "Gas and Electric Services":"Current provider" list box items are not in alphabetical order.

                              1. Contact Information: Form accepts invalid street address/city
                              2. Contact Information: Form accepts invalid email address
                              3. Contact Information: "NV" and "HI" is missing in "State" list box
                              4. Contact Information: Invalid item ("BC") present in "State" list box
                              5. Contact Information: User is not offered any further option to choose from when "Other" selected in "State" list box
                              Requirement says 50 states is available in the list. "DC" is out of the list?

                              6. Cell Phone Service: AT&T is missing in the "Current Provider" list box
                              7. Cell Phone Service: User is able to select both "I am happy with my current provider" and "I am open to using different provider"
                              8. Cell Phone Service: User is only able to select 0-10 in "Months left on current contract" list box
                              User should be able to select appropriate number of months. Example: 0-24 months

                              9. Gas and Electric Services: "open to" is typed as one word in "I am opento using different provider"
                              10. Pushing "Clear" button returns error message.
                              Error message: Error 17896 - Invalid data request. Please call technical support or restart your computer !!!

                                1. TAB order doesn’t work in order. It moves from «First name» to «Referred by» .
                                2. First name and last name can accept only digits
                                3. "Best way to contact me" - remove "me" and add list box to the same line with "Best way to contact"
                                4. "Zip code" field allows to use letters
                                5. remove "other" from list of state
                                6. Change "IND" on the list of state to "IN"
                                7.Remove BC from list of state
                                8. State list missing HI, NV states,
                                9. “Months left with the contract” include only 0-10 month, must be 0-12
                                10. "comments" box has no letter limitation
                                11. Button "clear" is not clear information but gives an Error 17896
