    "Contact Information":

    1. "First Name": When pressing TAB, the cursor should move to "Last Name" not to"Referred by".
    2. "States" listbox should not have "BC" and "Other" options.
    3. "States" listbox is missing the states of HI and NV.
    4. "Zipcode" should not accept non-digits.
    5. "Email" input should be limited to 255 characters.
    6. "Referred by" should not be marked as a required field.

    Section 1 "Cell Phone Service":

    7. "Current Provider" is missing "Other" option.
    8. "My monthly bill is approximately" should not accept non-digits
    9. "Minutes on plan": option "2100" is unneccessary.
    10. "Months left on current contract:" should have options up to 24 months.
    11. "Number of phones on plan": the number of options should go up to no more than 5.
    12. "I last upgraded my phone(s) approximately:" should have the word "ago" after "months".

    Section 2 "Local/Long Distances/International Service":

    13. "International calls included" and "Long Distance included" end with a "?" instead of ":".
    14. "Total Bill:" the input should be limited to 4 characters.

    Section 7 "Gas and Electric Services":

    15. "Current Provider:" the options should be listed in alphabetical order.
    16. The checkbox label has a spelling error with "opento" instead of "open to".

    17. Section 8. Field "Comments" is unnecessarily expandable.

    18. No field with default input focus.
    19. Sections 3 and 4: "Bundled with" should have the option "None".
    20. "My monthly bill is approximately" in ALL sections: input should be limited to 4 characters.
    21. "Current Provider:" listboxt in ALL sections: the options should be listed in alphabetical order.
    22. Buttons "Submit" and "Clear" should have the same font of the same size.
    23. Email should be listed as required information.
    24. The color of "*" symbol by field "Email" should stay red, not change to blue if the form is submitted incorrectly.
