    1. There is no Default Input Focus in the review.
    2."Contact Information "," Referred by" hasn’t the asterisk.
    3." Contact Information "," First Name" accepts no more than 30 characters instead of 31.
    4." Contact Information "," Last Name" accepts no more than 30 characters instead of 31.
    5." Contact Information "," Street Address" accepts no more than 100 characters instead of 255
    6." Contact Information "," Zip" accepts non digits as per requirements.
    7." Contact Information "," Email", accepts more than 255 characters
    8."Cell Phone Service" ,"Months left on current contract:". The word " More "is missed
    9."Cell Phone Service", "My monthly bill is approximately:" too much digits. Limit is 4.
    10. "Contact Information" TAB order is not correct. After the heading "First Name" it is logical to path the heading "Last Name" not to "Referred by"
    11. "Contact Information», the folder "Sate" contains 49 states instead of 50.
    12. It is possible to submit the review without any information in the box "Email" but it marked asterisk.
    13. "Local/Long Distances/International Service" TAB order is incorrect. It misses "Internet "and "Both" and goes straight to the "TV"

    Mac OS Sierra 10.12.5 / Safari 10.1.1
    1) «First Name:» text field, accepts 30 instead 31 characters.
    2) «Last Name:» text field, accepts 30 instead 31 characters.
    3) «Street Address:» text field, accepts 100 instead 255 characters.
    4) «State:» List Box offering the list of 49 states and item «other» instead of 50 states.
    5) «ZIP:» text field, accepts digits and letters and except as many characters as you copy/past by mouse .
    6) «Email:» text field, accepts 1063 instead 255 characters and includes all instead some special characters.
    7) «Phone:» 3 text fields, accept all characters copy/pasting by mouse instead digits only.

    8) ’Referred by’ field doesn’t have a asterisk.
    9) «Clean» button doesn’t clean the forms (error - invalid data request)
    10) Form doesn’t clean even after «refresh the page» on Safari browser
    11) Except form without Email (»Email» field does not aloud like required field, asterisk doesn’t change the color)
    12) «My monthly bill is approximately:» field except all characters and it include 11 characters (to much for this field)
    13) «Primary phone number:» field accept all characters copy/pasting by mouse instead digits only.

    Functionality testing based on requirements

    Section "Contact Information"
    #1. "First Name" text field accepts only 30 digits instead of 31
    #2. "Last Name" text field accepts only 30 digits instead of 31
    #3. "Street Address" edit box accepts only 100 digits instead of 255
    #4. "State" List Box contains only 49 states + "Other" string instead of the list of all 50 states.
    #5. "ZIP" text field accepts upper and lower case english letters instead of digits only.
    #6. "ZIP" text field accepts more then 5 characters, if they are not typed in but copy-pasted.
    #7. "Email" text field behaves like it is not required - the form can be submited without filling "Email" text field, and there is no expected error message.
    #8. "Email" text field accepts any characters, instead of "digits, letters, some special characters".
    #9. "Email" text field accepts up to 1063 characters, instead of only 255.
    #10. "Email" text field has no spell-checking for valid email address (like "ZIP" and "Phone" fields do).
    #11. "Phone" text field accepts any characters instead of digits only, if they are not typed in but copy-pasted.

    Section "1. Cell Phone Service"
    #12. "Current Provider" drop-down list does not contain the "Other" option like the rest drop-down lists on the page
    #13. Both checkboxes "I am happy with my current provider" and "I am open to using different provider" can be checked at the same time instead of disabling one another (can be replaced by radio buttons).
    #14. The check in the checkbox "I am happy with my current provider" does not disable the other elements in this section.

    #16. "Minutes on plan" drop-down list contains very small number of possible values - the client's plan may not match any of them.

    Section "8. Comments"
    #22. The field "8. Comments" has no limit for input symbols if they are not typed in but copy-pasted. Overfilling this field makes the webform crash when submitted.


    GUI testing based on GUI guidelines for WEB applications

    #23. Wrong tab order in the "Contact Information" section of the page: pressing Tab in the field "First Name" brings the cursor to the field "Referred by" and not to the field "Last Name" as logically intended.
    #24. No auto-tabbing is used in the "ZIP" field after entering 5 digits
    #25. "I am interested in Digital Phone Service (Unlimited Calling)" checkbox and its title are misaligned with the rest of similar elements on the page.
    #26. The "Submit" and "Clear" buttons have different size and are not aligned correctly.

    Energy Telecom Service Review bug report.

    #1. There is no of Default Input Focus on the page, it suppose to be on "First Name" field.
    #2. Contact Information: TAB order goes illogically, from the field "First Name" it goes to "Referred by". It suppose to go to "Last Name" field.
    #3. Local/Long Distances/International Service: TAB order goes illogically, from "Internet" radio button it goes to "Total Bill" text field. It supposed to go to "TV" radio button.
    #4. Local/Long Distances/International Service section: TAB order skipped all "No" radio buttons. It goes: from "Yes" to "Yes" and to "Yes" radio buttons order. It suppose to go from "Yes" radio button to "No" radio button.
