1. Open the Energy Telecom form page.
2. scroll to the bottom of the page.
Observe as the "Clear" button is not disabled by default when there's nothing to clear on the page. when the button is pushed, the page gives an error code.

Observe as the tab order moves the input focus to the "Referred by" data input field instead of correctly moving to "Last name" data input field.

#1 Inconsistency in section 4
It might be better to switch check boxes to radio buttons (as radio buttons are used mostly throughout the web page) for "I currently have:" in section "4. TV Service", and add "Both" option, so the approach is consistent.

Changing check boxes to radio buttons for "Current provider:" in section "1. Cell Phone Service" might be better 'cause by checking both boxes user can make a contradictory statement.

There is a "I am opento using different provider" mistype in the bottom of that section.

#4 "$" data input fields error
Input fields with "$" allow pasting of letterrs, when it should only let digits.

#5 Tab Sequence logic is broken
Instead of moving to the "Last Name" field (which would follow the left to right, top to bottom logic) Tab jumps to the "Referred by" field, what breaks the Tab Sequence logic.

#6 "State" drop down menu issue
Option "Other" follows the alphabetical order in the "State" drop down menu, however it'd be better to place it at the end of the list.

#7 "Email" field error
The website gives a warning on a missed field for all required fields, but the "Email" field when you press "Submit" button; asterisk on the "Email" field is blue when it has to be red.

Maybe change word "month" to the right of "I last upgraded my phone(s) approximately:" drop down menu (section "1. Cell Phone Service") to "month(s) ago", so it's clearer for users which information they need to give.

Check box option "I am interested in a wireless card for my laptop" doesn't logically belong to section "1. Cell Phone Service" (Cell Phone Service vs wireless laptop card).

The "Phone" field allows pasting of letters when it should not.

Not user friendly page visually and technologically

1. Inconsistency/ambiguous . Section “Contact Information” after auto filling “First Name” field , the field is highlighted in yellow (background-color: rgb(250, 255, 189), after typing this doesn’t happen.

“First name'' and “Last name” fields allows to enter numbers and characteristics and submit the form.
Error 17896 - Invalid data request. Please call technical support or restart your computer !!! - message when push “Clear” button.
Invalid “Input” button order after “First name”coming “Reffered by”, only after it “Last name”.
“Email” is marked as required, but accepting empty field.
“Reffered by” missing requarements mark.
“State” field have incorrect order and missing states, also state “Other”.
“Zip code” allows to input letters.
Cell Phone Current Provider missing “AT&T”.
“Months left to current contract” only up to 10.
“Enter” doesn’t ”Submit” Form.
“Submit” and “Clear’ buttons are have a different size.

Functionsl bugs:
First name field accepts blank values
Last name field accepts blank values
Form can be submited without email field being populated, even though email field is a required field
Zip code field accepts letters
Zip code field accepts invalid format "00000","12345"

GUI bugs:
Field "Best Way To Contact Me" is misaligned
ZIP box is misaligned in comparison with the page layout
Provider box - field grey out , misaligned and inactive
Dollar sign $ is tight toghether with the box. Requires space.
On empty page submit email is not highlighted in red
Checkbox "Wireless Aircard" is not the same as other checkboxes.

GUI checklist to the Energy-Telecom Online Form:
1. No default input focus.
2. No default input forces.
3. Tab order is not logical.
After the 2nd Tab push the order is not logical.
In section 3. High Speed Internet service The Tab order is skipping the "Provider"
4. The page doesn't provide default values
5. In section 7, there is a missing space in the last line between words "open" and "to"
6. In section 2, capitalized word Bill is unnecessary in the 5th line labeled Total Bill.
7. In section 1, label "current provider" doesn't provide option for "other"
8. In section 2, labels "International calls included?" and "Long distance included?" include unnecessary question marks.
9. When the "clear" push button applies, the error come out with following message "Error 17896 - Invalid data request. Please call technical support or restart your computer !!!'
10. in section 2, label "my monthly bill" is allowing letters. It should be digits only.
11. In section 2, label "month left on current contract" missing options 11 and 12. There are 12 months in a year.
12. Label "referred by" should be marked by asterisk * OR excluded from the list of * Indicates required field
13. The buttons "submit" and "clear" at the very bottom of the page have different font.
14. Label ZIP has inconsistency in abbreviation. IND has three letters abbreviation instead of two.
15. Label ZIP should have option "other" at the end of the list instead of alphabetical order.
16. Label ZIP should exclude the first abbreviation of BC since it's not a state of USA
17. Label ZIP is missing following states of USA: DC and HI
